AdventHealth Partners with Aidin to Enhance Care Coordination

What You Should Know: 

– AdventHealth, a healthcare system with over 50 hospitals across nine states, today announced a partnership with Aidin, a healthcare technology company specializing in care coordination solutions. 

– The collaboration will leverage Aidin’s innovative platform to enhance AdventHealth’s care management processes, improve patient outcomes, and support the transition to value-based care.

Advance Value-Based Care Across Health System

As the healthcare industry shifts towards value-based care models, effective care coordination is essential for improving patient outcomes and reducing costs. Aidin’s platform provides tools and automation to streamline workflows, enhance communication, and optimize care transitions across the healthcare continuum.

By implementing Aidin’s platform, AdventHealth’s care management teams will be able to:

  • Reduce length of stay (LOS) and readmissions: Optimize discharge planning and care transitions to minimize unnecessary hospitalizations.
  • Increase hospital bed capacity: Efficiently manage patient flow and optimize bed utilization.
  • Improve post-acute care selection: Identify the most appropriate and highest-quality post-acute care settings for each patient.
