What You Should Know: 

– HealthEdge, a provider of next-generation core administrative processing systems (CAPS) and care management solutions, and Unite Us, a technology partner for social care transformation, announced a strategic partnership to empower health plans with the tools to deliver seamless, whole-person care at scale. 

– The strategic partnership addresses the Social Determinants of Health (SDOH) needs of members by leveraging the SMART on FHIR interoperability standard.

Integration Empowers Health Plans to Deliver Whole-Person Care at Scale

As part of the partnership, HealthEdge’s GuidingCare® platform will integrate with the Unite Us platform to connect members with essential SDOH resources, such as food and housing services. This innovative partnership has the potential to connect over 20 million U.S. health plan members with the social services they need more efficiently.

Addressing SDOH Needs in Care Management

Social determinants of health (SDOH) play a crucial role in an individual’s overall well-being, impacting health outcomes and quality of life. Factors such as access to food, housing, transportation, and social support can significantly influence a person’s health.

Recognizing the importance of addressing SDOH, healthcare organizations are increasingly seeking ways to integrate social care into their care management programs. This partnership between HealthEdge and Unite Us provides a solution that enables health plans to:
