Emory Healthcare Launches Population Health Collaborative with Guidehealth

What You Should Know:

–Emory Healthcare, Georgia’s most comprehensive academic health system, and Emory Healthcare Network, the health system’s clinically integrated network, today announced a new population health collaborative (PHC) with Guidehealth, a nationally acclaimed, AI-enabled healthcare and value-based managed services company. 

– This entity integrates Emory’s growing primary care service line and affiliate physician network to expand value-based care (VBC) – a focus on quality and outcomes while containing costs – to more than 350,000 individuals across Georgia. Emory Healthcare’s Population Health Collaborative is aligning with Guidehealth to accelerate performance of quality of care and overall health outcomes across its clinics and network.

Advancing Comprehensive Care Through Emory Healthcare and GuideHealth Collaboration 

Emory Healthcare, the most comprehensive academic health system in Georgia, operates with nearly 24,000 employees across 10 hospitals and 490 provider locations. With 2,796 licensed patient beds and a network of more than 4,000 physicians practicing in over 70 specialties, Emory serves metro Atlanta, Georgia, and beyond. Its reach extends through partnerships with six regional affiliate hospitals, St. Francis–Emory Healthcare in Columbus, and a clinically integrated physician network. 

 Expanding Access and Preventive Care 

Through an initiative involving over 500 Emory and affiliate primary care providers, the system delivers patient care, education, research, and community engagement at various locations. The program focuses on annual patient visits to: 

– Assess health needs and identify risk factors. 

– Detect chronic conditions—such as diabetes, hypertension, and hyperlipidemia—at early stages through lab work and screenings. 

– Implement preventive strategies to address potential health concerns before they advance. 

The collaboration with GuideHealth enhances access and connectivity for Emory clinicians dedicated to improving health outcomes and achieving quality metrics. 

Integration of Innovative Care Management 

GuideHealth, recognized as Best in KLAS for Value-Based Care Managed Services for six consecutive years, brings its award-winning platform to Emory. Key features include: 

– Clinical support services integrated directly into provider workflows. 

– Tools to enhance care coordination, reduce administrative burdens, and address care gaps cost-effectively. 

– Healthguides™, a virtual team assisting Emory providers in extending practice reach and optimizing care for complex patients and families.
