ReallyWell: Transforming Health and Wellness with Modular, Scalable, and Integrated Solutions

ReallyWell is a modular, flexible, and scalable platform designed to ensure seamless and efficient implementation across various healthcare settings. It integrates effortlessly with any plan, provider, or employer experience and supports third-party vendors and point solutions. Custom configuration options allow organizations to meet specific operational and customer objectives. To cater to diverse populations, ReallyWell offers three care options: high-touch, assisted, and autonomous. Wellbeing managers can autonomously configure and launch programs at scale through an intuitive administrative portal.

The platform delivers clinically prioritized wellbeing journeys that guide each member toward better health, providing timely and relevant content that adapts dynamically to health status changes. As the first platform to integrate clinical support, wellness services, and contextual navigation across the entire continuum of care, ReallyWell empowers patients and members with one-click omnichannel access to nurses or health coaches. This immediate access eliminates healthcare navigation challenges, supporting proactive health management through early awareness and timely interventions.

Key Innovative Features Driving Engagement:

1. WellBeing CoPilot: ReallyWell consolidates and synchronizes various data points—including healthcare interactions, claims, advanced biometrics, HRAs, clinical risk factors, SDOH attributes, and behavioral needs—into a unified data profile. Using these insights, the GenAI-powered WellBeing CoPilot delivers highly contextualized content, personalized health and wellbeing recommendations, and a “next-best-actions” care journey, creating a relevant, real-time engagement experience for each member.

2. Health Risk Assessment (HRA): ReallyWell’s NCQA-accredited Health Risk Assessment features an enhanced question set that reduces completion time, resulting in a 59% completion rate. This HRA helps guide members to the appropriate level of care at the right time and can be customized for various data-gathering needs, such as identifying health issues impacting high-risk groups to improve population health.

3. Dynamic Wellness Score: This proprietary calculator generates an overall wellness score and component-specific scores for each member. Displayed visually, the score updates in real-time based on the member’s actions, providing tangible progress and keeping them informed of their wellbeing journey’s status.
