Humana, Interwell Health Expand Value-Based Kidney Care Agreement to Florida

What You Should Know: 

– Humana Inc., a health and well-being company, and kidney care management company Interwell Health are expanding their value-based care agreement to include Florida. 

– The expansion will provide eligible Humana Medicare Advantage members living with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) in Florida with access to Interwell’s comprehensive care and specialized resources.

Value-Based Care for Improved Outcomes

Value-based care is a patient-centered model focused on improving patient health outcomes and quality of life through coordinated efforts. With this expanded agreement, members in the program will have access to Interwell’s network of 2,000 nephrologists, renal care coordinators, and in-home virtual support from a multidisciplinary team.

Humana’s Success in Value-Based Kidney Care

Humana’s commitment to value-based care partnerships has shown promising results, including a 5% reduction in unnecessary hospital admissions in 2023. One notable success story is that of Mary Johnson, who received a successful pre-emptive kidney transplant thanks to the support of the Interwell care team.

Expanded Access to Specialized Care

With this expansion, Interwell’s value-based support services are now available in 14 states for eligible members with CKD and in 39 states for those with ESKD. This increased access to specialized care is critical for improving the lives of individuals living with kidney disease.
