– Florida Medical Clinic, a large physician enterprise in northern Florida, experienced a 58% decrease in long-term acute care hospital ACO spend, an 18% reduction in SNF episodes, a 2.3-day reduction in SNF length of stay, and an 8% decrease in ACO SNF readmissions to hospital.
