What You Should Know: Trualta, a family caregiver education and support platform, and PointClickCare, a cloud-based healthcare platform for long-term and post-acute care (LTPAC) providers, have announced a new partnership to provide crucial support to families caring for loved ones after discharge. Under this new agreement, Trualta will be offered as an integrated partner solution to PointClickCare customers through their online Marketplace. This means care facilities using PointClickCare can easily provide Trualta's resources to families at the crucial discharge stage. Empowering Families for Smooth Transitions Prior to discharge, families will be invited to access Trualta's comprehensive library of articles, videos, and modules covering various caregiving topics. These resources include essential information on: - Preparing for discharge: Understanding medication management, home safety adaptations, and equipment needs. - Transitioning from hospital to home: Managing daily routines, handling potential complications, and accessing community support. - Life after discharge: Building resilience, navigating emotional challenges, and finding caregiver support groups. Furthermore, Trualta offers virtual support groups where families can connect, share experiences, and ask questions in a safe and supportive environment. Proven Impact on Patient Outcomes This partnership holds significant potential to improve care transitions and patient outcomes. According to a recent Trualta study, 30 days of Trualta use can decrease annual unexpected hospital visits among care recipients by 20%. This translates to improved health outcomes, reduced healthcare costs, and greater peace of mind for both families and providers. "We are so excited that more family caregivers will be able to receive proper education and support as their loved ones transition home from care facilities," said Jonathan Davis, Trualta CEO. "We know that supporting families through discharge improves provider satisfaction rates and reduces hospital readmissions."
