This means care facilities using PointClickCare can easily provide Trualta's resources to families at the crucial discharge stage. Empowering Families for Smooth Transitions Prior to discharge, families will be invited to access Trualta's comprehensive library of articles, videos, and modules covering various caregiving topics. These resources include essential information on: - Preparing for discharge: Understanding medication management, home safety adaptations, and equipment needs. - Transitioning from hospital to home: Managing daily routines, handling potential complications, and accessing community support. - Life after discharge: Building resilience, navigating emotional challenges, and finding caregiver support groups. Furthermore, Trualta offers virtual support groups where families can connect, share experiences, and ask questions in a safe and supportive environment.
