– Discharge Summaries is designed to address the challenges of patient discharge communication by providing care coordinators with easy access to real-time discharge information, including medication lists, follow-up appointments, and other vital details. This information is critical for ensuring that patients receive the appropriate care after they leave the hospital and can help to reduce readmissions.

Crucial Insights During Patient Transitions

Manual patient discharge reports, containing vital information like medication lists and follow-up appointments, often face challenges in digital access due to proprietary Electronic Health Record (EHR) systems. The Discharge Summaries solution by Bamboo Health addresses communication gaps, providing real-time access to essential patient data and adding clinical context to Pings alerts.

Benefits of Discharge Summaries include:

Seamless Access: Care coordinators can effortlessly obtain discharge information, avoiding manual tracking and phone calls.

Unified Platform: Pings platform now includes access to hospital discharge reports, regardless of transmission method, alongside real-time alerts for Admissions, Discharges, and Transfers (ADT).

Enhanced TCM Billing: The solution supports Transition of Care Management (TCM) billing opportunities with comprehensive clinical context.

Network Scale: Discharge Summaries are available from in-network or out-of-network facilities nationwide, and through interoperability networks such as Commonwell or Carequality Alliance.

Readmission Reduction: Early capture of essential details minimizes avoidable emergency department visits and readmissions.
