Overall, Form Health patients lose up to 15% of their body weight at three months and up to 20% at six months.

Form Health is currently available in 21 states across the US including CA, IL, NY, PA, TX, and VA. After initial consultation, the monthly program costs $99 and Form doctor visits may be covered by Aetna, Anthem, Blue Cross Blue Shield, Cigna, Humana, UnitedHealthcare, and others at little or no cost.
