CarePort Health, Jackson Health Launch First-of-its-Kind Integration with Cerner EHR System

CarePort Health, a provider of care management and post-acute outcomes solutions, announced that longtime Care Management client Jackson Health System will continue its partnership and extend its agreement to utilize CarePort’s discharge planning and referral management platforms at six Miami-area hospitals, medical centers and long-term care facilities.

Additionally, CarePort and Jackson Health System developed a first-of-its-kind integration solution between the hospital’s Cerner EHR and the CarePort Care Management platform. The solution improves end-user workflow and enhances efficiency while providing a more seamless experience for staff to deliver the right care at the right time. This partnership helps Jackson Health System further expand its already-impressive quality of care services for residents of Miami-Dade County.

The newly created integration solution allows users to launch directly into the referral creation workflow within CarePort via single sign-on (SSO) from the Cerner application while keeping patient data accessible throughout both systems. The integration allows users to view and identify specific clinical data to create a referral specific to the patient’s clinical needs. 

Another key element of this bidirectional integration was to ensure that the final location of the patient’s placement is sent back to Jackson Health System’s Cerner EHR, thereby completing the referral process and increasing the visibility for providers across the care continuum. Jackson Health System is the first CarePort client to establish this level of integration between a Cerner EHR system and CarePort’s discharge planning app.

An integrated healthcare delivery system, Jackson Health System consists of its centerpiece, Jackson Memorial Hospital, Jackson South Medical Center, Jackson North Medical Center, Holtz Children’s Hospital, Jackson Rehabilitation Hospital, Jackson Behavioral Health Hospital; a network of UHealth Jackson Urgent Care centers; multiple primary care and specialty care centers; and two long-term care nursing facilities. The new Jackson West Medical Center campus is under development in the City of Doral and will feature adult and pediatric emergency rooms and outpatient services.
