Mount Sinai to Deploy Lumeris Value-Based Services to Optimize Population Health Outcomes

Mount Sinai Health System, New York City’s largest integrated delivery system has signed an agreement with Lumeris, a value-based care management service provider to optimize Mount Sinai’s population health outcomes. Mount Sinai has already made great strides in covering populations at risk but under the guidance of Lumeris they plan to double the number of lives covered under value-based care arrangements.

The multi-year collaboration focuses on better engaging physicians in the value-based care offering to improve care management and clinical practices –while driving better outcomes. Mount Sinai chose Lumeris because of the company’s proven expertise and because they wanted the most “innovative collaborators to help support us in this journey,” and because “Lumeris will accelerate our ability to effectively and responsibly manage large populations in risk-based models.” 

Lumeris serves as a long-term operating partner for healthcare organizations that are committed to the transition from volume- to value-based care and delivering extraordinary clinical and financial outcomes. The company’s value based care management platform guides health systems and providers through seamless transitions from volume to value, enabling them to deliver improved and more affordable care across populations—with better outcomes.
