Holy Name Medical Center to Implement CareCloud’s EHR/PM Across 35 Ambulatory Practices

Holy Name Medical Center, a Teaneck, NJ-based not-for-profit healthcare organization has selected CareCloud to implement its EHR and practice management across 35 ambulatory medical practices. Holy Name has a history of developing their hospital health technology in-house and currently has a team of computer scientists and engineers, who support a proprietary EHR system for their inpatient services.

Holy Name has seen rapid growth through its physician network, Holy Name Medical Partners and partnered with CareCloud to help them successfully integrate and manage their large, complex and growing set of ambulatory practices. As part of the new EHR/PM impelementation, CareCloud is supporting the medical center as teams prepare for the data portability requirements of working with accountable care organizations (ACOs). Holy Name also is leveraging CareCloud’s population health management (PHM) capabilities powered by Lightbeam Health Solutions to identify and engage patients with gaps in care to improve quality and outcomes.

Working in conjunction with Holy Name’s revenue cycle management partner, Pacific Global, CareCloud will also help streamline financial and clinical workflows within Holy Name’s ambulatory practices.
