Bellin Health, an integrated healthcare delivery system based in Green Bay, WI has ) announced a new partnership with PatientPing, a Boston-based technology company building a national care coordination network. The new partnership will help Bellin Health collaborate on shared patients with other providers outside their network and across the care continuum, enabling a team-based, coordinated approach to patient care.
Bellin is comprised of Bellin Hospital, Bellin Psychiatric Center, 32 Bellin Health primary care physician clinics, and several retail health clinics known as Bellin Health FastCare. Bellin also operates Bellin Health Oconto Hospital, a critical care access hospital in Oconto, Bellin Fitness, and Bellin College.
Upon joining PatientPing, providers receive real-time notifications, or Pings, whenever their patients receive care, as well as patient stories that provide critical patient context such as attributed provider, visit histories and care program affiliation. By receiving real-time information about a patient’s medical history, healthcare team providers can engage and coordinate during the patient’s healthcare event, helping to prevent duplication of services, reduce avoidable readmissions and ensure safer care transitions.
