Cordata Healthcare Lands Nearly $3M for Care Coordination Platform

Cordata Healthcare Innovations, a Cincinnati, OH-based provider of specialty care coordination platforms has landed nearly $3 million in Series A funding led by Chicago-based First Health Capital Partners (FHCP) with participation from CincyTech. The company plans to utilize the funding to build on market momentum and scale the capabilities of its multi-specialty platform for care coordination.

Founded in 2014, Cordata Healthcare Innovations offers specialized software applications for effective patient management using patient-focused, nurse navigation models. It offers Cordata Spine, Cordata Oncology, Cordata Consensus, and Cordata ConsensusNow, products which are designed to assess patient needs prior to appointment scheduling to ensure appropriate utilization of clinicians and other clinical resources.

Key features of the platform include:

– Focus and streamline screening and evaluation of high-risk patients and populations

– Direct interfaces and integration with other clinical information systems- patient information, scheduling EHR/oncology EHR, pathology, imaging reporting system

– intuitive tools for communication with and reminders for patients and clinicians

– Flexibility to support any cancer types and unique workflows/processes – real-time and custom reporting and treatment lag times, navigator activity, patient status, surgical utilization/type, diagnoses per month

Cordata has recently landed new or renewed contracts with Florida Hospital Cancer Institute, Dignity Health and Texas Back Institute, among other hospitals.
