Florida’s PremierMD ACO Selects eClinicalWorks for Population Health Management

eClinicalWorks Named Most Used System Across ACOs for PHM

Florida’s PremierMD ACO LLC (Premier), an ACO participating in the Medicare Shared Savings Program, has chosen the eClinicalWorks CCMR population health platform to advance its ACO objectives, coordinate care across the continuum as well as evaluate population health and quality improvement activities. eClinicalWorks will also be the preferred EHR solution for the organization.

Premier participates in the Medicare Shared Savings Program, which has been created by the Centers for Medicare & Medicaid Services (CMS) for facilitating coordination between providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries and reduce the total cost of care.

eClinicalWorks CCMR will provide a single vendor solution, integrating analytics and care coordination together on a single, unified platform. The technology will assist Premier in meeting its quality measures and performance metrics in addition to helping facilitate the Patient Centered Medical Home (PCMH) certification process in which Premier’s practices are engaged.

“Premier focuses on developing and supporting practices that promote better care for patients,” said Girish Navani, CEO and co-founder of eClinicalWorks. “eClinicalWorks will help Premier’s physicians gain deeper insight and understanding of patient populations and enhance decision-making at the point of care. eClinicalWorks CCMR will provide Premier with visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including CMS, to provide cost and utilization for services delivered to patients. By also using messenger capabilities, communication between providers and patients is simplified, fast and effective for aspects including appointment reminders and preventative care measures.

Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health effectively. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
