Deloitte and NNEACC collaborate to offer value-based care solution called “Insight” that will help lower costs and manage the risks of population health.
Deloitte Consulting has announced a groundbreaking collaboration with the Northern New England Accountable Care Collaborative (NNEACC) and will be offering a solution called “Insight” that will help U.S. health care providers make the shift to a health system defined by population health and risk-based contracts.
ConvergeHEALTH’s data integration, analytic and workflow technologies is leveraged when combined with NNEACC’s proprietary predictive models, stratification algorithms and outcomes research capabilities to help
- improve population health management outcomes
- reduce unwarranted variation in health care
- improve care coordination
- leverage evidence-based guidelines
- manage clinical and financial risk
Further complementing this tool is ConvergeHEALTH’s recently launched Intellect along with PopulationMiner and OutcomesMiner, two additional solutions that provide a deeper understanding of the relationship between personalized therapies and disease states.
“The platform is designed to integrate and analyze EMR, claims, HIE and operational data to enable member health systems to coordinate patient care, drive provider performance improvements and engage patients to achieve the health care triple aim of better care, better health and at lower cost,” said Jason Girzadas, national managing director of life science and health care, Deloitte Consulting LLP.

The NNEACC solution is already operating in Vermont, New Hampshire and Maine via a SaaS-based model that facilitates benchmarking and collaboration among its members to identify and implement best practices for care delivery. The solution is offered as both a subscription and on premise option for health systems.
