– The strategic partnership integrates Deloitte’s advanced data analytics with the extensive Unite Us network of health and social care providers. The partnership seeks to proactively identify needs, connect individuals with essential services, and improve outcomes for government and healthcare programs.

Redefining the ‘Last Mile’ of Care

The collaboration is designed to bridge the final, critical gap between individuals and the resources they need. By combining their respective strengths, the two organizations aim to create a more seamless and effective support system.

The partnership merges powerful technology platforms with a vast, outcome-focused provider network. It brings together Deloitte’s innovative solutions, such as PeoplePrism™ and the GovConnect™ Community Resource Engine, with Unite Us’s real-time, cross-sector coordination technology and its Community Resource Data Infrastructure.

This integrated approach enables state and local agencies, healthcare organizations, and community partners to:

  • Quickly identify gaps in social care.
  • Seamlessly connect people to services for housing, veteran support, mental health, and more.
  • Monitor progress and track outcomes in real time.
