U.S. Preventive Medicine, Solera Health Partner to Deliver Comprehensive Health Coaching

Solera Health, today announced its partnership with U.S. Preventive Medicine (USPM) to deliver its comprehensive health coaching and nurse based chronic condition care management services within the framework of the Centers for Disease Control and Prevention (CDC) National Diabetes Prevention Program (DPP) for Solera clients. USPM’s health coaching lifestyle management and nurse based chronic care management programs uniquely combine innovative wellness programs focused on prevention with lifestyle management and disease/care management. All programs have generated significant and sustained positive clinical outcomes that have been independently validated.

Solera connects patients, payers and physicians to a network of partners who are preventing and managing chronic disease. The company coordinates with major health plans in targeting people at risk of developing type 2 diabetes and connects them with certified DPP service organizations like USPM. Solera and USPM are committed in this partnership to promote and enhance patient access, program quality, and sustainability of the National DPP to help prevent the CDC-estimated 86 million Americans with prediabetes from developing diabetes.

According to the CDC, prediabetes affects approximately one out of three American adults and 90% of the people with prediabetes don’t know they have it. The condition can put people at increased risk of developing type 2 diabetes, heart disease and stroke; the majority of which can be prevented or delayed through evidence-based lifestyle change programs and interventions. Diabetes is a growing epidemic with someone being diagnosed every 21 seconds. The total cost of diabetes and prediabetes in the U.S. is estimated to be $322 billion dollars and people with diabetes have health care costs that are 2.3 times greater than those without diabetes.
