Livongo Launches Digital Diabetes Management Platform for 4,000 Alaskans

Livongo Health Raises $52.5M to Expand Diabetes Management to International Markets

.Livongo, a consumer digital health company has teamed up with Medical Park Family Care, the largest primary care group in Alaska to launch Livongo for Diabetes platform to 4,000 people with diabetes receiving care from Medical Park Family Care. The partnership will enable providers to recommend Livongo’s digital health platform, alongside diabetes prescriptions, regular primary care appointments, and clinic services, to ensure that people with diabetes receive 24/7 ongoing support and tools to better self-manage diabetes.

Medical Park Family Care operates a diabetes center of excellence where 90% of their people with diabetes receive care, and they are now expanding their services offered to include Livongo’s remote-monitoring and lifestyle modification coaching. Livongo is redesigning chronic condition management, starting with diabetes. Livongo delivers a personalized experience, using data and clinical science, and helps to make staying healthy easier, which results in measurable positive health outcomes for people with diabetes1 and lower costs for payers2. Combining Livongo’s remote monitoring and coaching approach with Medical Park gives people a seamless experience between their provider and their time outside the office.
