Summa Health System and Humana Forms ACO in Akron

Summa Health System’s accountable care organization (ACO) – NewHealth Collaborative and Humana have formed a new ACO agreement to improve health and more coordinated services for the growing number of Humana Medicare Advantage (MA) members in Akron. The new agreement will include value-based incentives tied to performance improvement and will offer more personalized care and a broad range of population management tools like, predictive analytics and chronic care, disease management and wellness programs.

The agreement will also emphasizes quality outcomes with a focus on care coordination and leverages measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) for things such as diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication.

Summa Health System is one of the largest integrated health care delivery systems (IDN) in Ohio, curretly serving more than a million patients each year. NewHealth Collaborative, a clinician-led collaborative that partners with communities is the only ACO in Ohio to save money from its participation in the Medicare Shared Savings Program. 

Humana has become a strong national accountable care player by working closely with doctors and hospitals through its Accountable Care Continuum. The new value-based agreement with Summa Health System is a stepped approach to pay-for-value reimbursement that allows Humana to implement different accountable care approaches with physicians based on each groups’ practice complexity and infrastructure capabilities.
