MedeAnalytics and ZeOmega Partner to Launch Population Health Navigator

What You Should Know: 

– MedeAnalytics, a healthcare enterprise data enrichment platform and analytics software-as-a-service (SaaS) leader, and ZeOmega®, a population health management organization, today announced the co-development of Population Health Navigator (PHN). 

– The new solution unites predictive insights with care coordination to power value-based care programs. PHN combines the analytical capabilities of MedeAnalytics’ Health Fabric™ with ZeOmega’s care management engine, enabling payers and providers to query their data, visualize results, and take action – all within a single, integrated platform.

Driving Actionable Insights for Value-Based Care

Amidst increasing pressure to lower medical loss ratios (MLRs), improve outcomes, and manage complex populations, many healthcare organizations still rely on fragmented systems that isolate analytics from care delivery. PHN addresses this challenge by establishing a “single source of truth” that fosters collaboration across clinical, operational, and financial teams.

PHN is designed to make healthcare data more actionable. By providing a unified platform, healthcare organizations can significantly reduce adverse events such as emergency department visits and readmissions by 10-15%, which translates into millions in savings and measurable ROI for both payers and providers.

PHN will be offered as a configurable orchestration layer within ZeOmega’s Jiva platform. This integration is expected to enable faster interventions, more targeted care, and a lower total cost of care, while simultaneously meeting evolving regulatory, clinical, and consumer demands.
