Elation Health Partners with SMG to Deliver Custom EHR for 5,000+ Physicians

(SMG), the nation’s largest Korean-American-focused Independent Practice Association (IPA) serving more than 100,000 patients through a robust network of over 5,000 primary care physicians and specialists across seven U.S. markets. 

– Through this partnership, SMG’s affiliated primary care practices will gain access to a customized electronic health record (EHR) platform from Elation Health, designed to enhance care delivery, improve care coordination, and support clinical integration to drive better outcomes.

Seoul Medical Group and Elation Health Partner to Advance Value-Based Care for Independent Practices

Seoul Medical Group (SMG), a nationally recognized network of independent community physicians, has announced a new partnership with Elation Health to expand access to value-based care tools across its provider network. The collaboration is designed to equip independent practices—particularly those serving Asian American populations—with integrated digital infrastructure, tailored support, and scalable workflows that enhance clinical outcomes and operational efficiency.

Founded in 1993, SMG operates one of the country’s largest physician-led independent practice associations (IPAs), with a network of 4,800 primary care and specialist providers committed to delivering comprehensive care that extends beyond clinical needs. The group is deeply focused on improving health outcomes in underserved communities, with a particular emphasis on culturally attuned care for Asian American patients.

Under the partnership, participating SMG practices will benefit from:

  • Seamless digital integration with Elation’s EHR platform and SMG’s broader network;
  • Tailored onboarding and personalized, ongoing support to ensure a smooth transition;
  • Competitive pricing on Elation’s solutions to reduce the burden of technology costs;
  • Custom workflows and templates aligned with SMG’s value-based care protocols, including automated reporting features that ensure providers receive credit for delivering high-quality care.

This collaboration empowers clinicians to more efficiently manage their attributed SMG patient panels while maintaining the personalized, community-centered approach that defines independent practice. By automating the submission of SMG-required documentation and deploying pre-configured templates, physicians can focus on care delivery rather than administrative demands—particularly in Medicare Advantage and senior care populations.
