IBX Medicare Advantage Members Gain Access to Virtual Brain Health and Dementia Support

What You Should Know:

–  Independence Blue Cross (IBX) knows that the brain is at the center of everything a person does, and that keeping it healthy is essential to their well-being. Keeping this and the cognitive well-being of its members at the forefront, IBX has teamed up with Isaac Health, a virtual-first clinic that provides brain health and dementia care.

–  Through this new relationship, a subset of IBX Medicare Advantage members who have a diagnosis of dementia or who are at risk for dementia have access to Isaac Health’s medical and support services.

Coordinated Brain Health Support for IBX Medicare Advantage Members through Isaac Health

Independence Blue Cross (IBX) Medicare Advantage members with dementia or at risk for cognitive decline can now access specialized brain health services through a partnership with Isaac Health. Eligible members may be referred through their primary care physician or specialist. Additionally, members engaged with IBX Case Management—particularly those with a dementia diagnosis or at high risk—may be proactively contacted by Isaac Health’s team.

Isaac Health outreach specialists clearly identify themselves as calling on behalf of IBX and begin by verifying the member’s identity to ensure privacy and security. A brief screening follows to assess the need for further evaluation. If cognitive changes are identified in two or more areas during this initial assessment, members are offered the opportunity to schedule a virtual follow-up consultation with an Isaac Health provider.

According to Dr. Luz Ramos, geriatrician and medical director at IBX, this collaboration supports early identification and intervention for cognitive decline. “Working with Isaac Health enables us to connect members with the resources they need while empowering them to take an active role in managing their brain health. This is increasingly vital, as projections from the National Institutes of Health indicate that new dementia cases in the U.S. are expected to double by 2060.”

Isaac Health delivers comprehensive, personalized care for individuals with dementia and related cognitive conditions. Services are designed to enhance both patient and caregiver quality of life and include:

  • Comprehensive assessments and specialist consultations for brain health;
  • Neuropsychological testing, laboratory work, and imaging;
  • Individualized care plans incorporating cognitive therapy or cognitive behavioral therapy;
  • Medication management and adjustments;
  • Follow-up visits with a multidisciplinary team comprising neurologists, geriatricians, psychiatrists, and psychotherapists;
  • Skills training to support patient independence in daily activities;
  • Access to support groups and referrals to local community resources;
  • A caregiver support hotline and curated resources to assist family caregivers.

To ensure accessibility, all consultations are conducted via telehealth for members with internet access. For those requiring additional assistance or lacking internet connectivity, Isaac Health arranges for an in-home assistant to facilitate the virtual consultation.

This integrated approach enables IBX Medicare Advantage members to receive timely, expert-led cognitive care, helping them navigate brain health challenges with dignity and support.
