Modivcare and Aetna Better Health of Illinois Launch Community-Based Smart Health Stations to Advance Preventive Care and Equity
Modivcare Inc. What began with five community organizations is now expanding to 15, creating convenient, no-cost access points where individuals can:

- Complete health risk surveys and receive personalized care recommendations
- Identify unmet social and healthcare needs
- Connect with relevant community and health plan resources
These Smart Health Stations are placed in trusted community hubs, extending healthcare reach to where individuals live and work. Aetna complements this effort by partnering with local organizations to provide its Medicaid members with tools that support self-awareness, education, and timely access to care services.
