Zing Health & Strive Health Expand Access to Value-Based Kidney Care

– Strive Health, a national leader in value-based kidney care, and Zing Health, a leading Medicare Advantage insurer, have announced the expansion of their existing partnership to deliver specialized kidney care to all eligible members across Zing’s geographical footprint.

–  Strive will now support Zing members in Ohio, Tennessee and Mississippi and continue coverage in Illinois, Indiana and Michigan. Through this expanded partnership, Strive is projected to serve thousands of Zing members with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) in 2025, with double-digit growth expected this year.

Strive Health and Zing Partner to Enhance Value-Based Kidney Care

Strive Health, the nation’s leading provider of value-based kidney care, partners with innovative healthcare payors and providers to deliver integrated, technology-enabled care for patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). Through risk-based payment models, Strive collaborates with commercial and Medicare Advantage payors, Medicare, health systems, and physicians to improve patient outcomes while reducing costs. Currently, Strive supports over 121,000 CKD and ESKD patients across all 50 states and works with more than 6,500 providers. Its case management and population health programs hold accreditation from the National Committee for Quality Assurance (NCQA), while its technology platform, CareMultiplier™, is HITRUST-certified.

Each year, over 130,000 Americans are diagnosed with CKD, a condition responsible for 24% of annual Medicare spending and often associated with fragmented, high-cost care. To address this challenge, Strive has partnered with Zing to integrate specialized nephrology care with Zing’s tailored Medicare Advantage benefits, ensuring seamless, coordinated care for CKD patients.

The collaboration leverages:

  • Strive’s value-based care model, which focuses on slowing disease progression through predictive analytics, interdisciplinary clinical teams, and high-touch patient engagement
  • Zing’s Medicare Advantage plans, designed specifically for individuals with chronic conditions, addressing both medical and social determinants of health
