PointClickCare Launches Pharmacy Connect for Senior Living Medication Management

– PointClickCare, a healthcare technology platform launches its Transitions of Care (TRC) solution at the RISE National Conference. 

– The solution aims to help Medicare Advantage plans improve their TRC Star measure performance by automating the access and delivery of critical admission and discharge information.

Automating Data Exchange for Timely Interventions

PointClickCare’s TRC solution is designed to streamline post-discharge coordination by automating the real-time access and exchange of admission and discharge data between payers and providers. By leveraging existing provider workflows, the solution ensures the timely delivery of required information, meeting the 48-hour time-bound specifications of the CMS TRC Star measure’s “Notification of Admission” and “Receipt of Discharge Information” components.

The new solution addresses the significant issue of hospital readmissions among Medicare beneficiaries. With nearly 20% of patients readmitted within 30 days and 34% within 90 days, the financial burden on Medicare is substantial, costing $17.4 billion annually. Coupled with the Centers for Medicare & Medicaid Services’ (CMS) increased focus on seamless care transitions, Medicare Advantage plans are under pressure to intervene quickly and effectively to prevent avoidable readmissions.

This new solution builds upon PointClickCare’s established track record of improving patient outcomes through connected care ecosystems. By automating data exchange and streamlining workflows, the TRC solution empowers Medicare Advantage plans and their provider networks to deliver more efficient and effective care transitions.
