Story Health is a digital health company dedicated to transforming specialty care delivery. With a technology-enabled and human-powered approach, Story Health aims to revolutionize the care journey for patients with heart disease by enabling clinicians and institutions to implement and scale continuous specialty care for every patient, ensuring no one is left behind. Collaborating with prestigious health systems across the country, including ChristianaCare, Intermountain Health, and Saint Luke’s Health System, as well as leading at-risk providers, Story Health is at the forefront of innovative healthcare solutions.

Key aspects of Story Health’s approach include:

– Integrated Technology and Specialized Care Teams: Combines advanced technology with a dedicated team of cardiologists, cardiac nurse practitioners, registered nurses, and health coaches to improve health outcomes and quality of life for individuals with cardiovascular issues.

Founded in 2019 by two African American physicians and experienced healthcare executives, Zing Health focuses on addressing inadequacies in the healthcare system through collaborative, community-based Medicare Advantage plans. These plans are tailored for seniors with chronic conditions and aim to reduce healthcare disparities by addressing social determinants of health.
