San Mateo County Health Selects Innovaccer’s Healthcare AI Platform

–  Innovaccer Inc., a leading healthcare AI platform company, today announced that San Mateo County Health (SMCH) has selected Innovaccer’s Healthcare AI Platform and population health management solutions to modernize their technology infrastructure and improve patient care services across the county.

– SMCH is dedicated to promoting healthy living for all residents of San Mateo County. It provides healthcare services in the county through eleven divisions, nine of which provide a wide range of clinical care, family and adult social services, and regulatory oversight and public health services.

Enhancing Patient Care with Advanced Data Integration: San Mateo County Health Partners with Innovaccer

San Mateo County Health (SMCH) is dedicated to administering public health programs and providing clinical and supportive services to the community. Their mission is to help every individual in San Mateo County live longer and healthier lives. SMCH strives to protect and promote the health of its residents by preventing diseases before they occur, treating mental and physical conditions, and monitoring both the environment and the community’s overall health.

To achieve these goals, SMCH sought a platform that could create a unified patient record, enabling advanced analytics and integrated workflows. Improved access to patient data at the point of care would enhance collaboration among care teams and ensure quality healthcare is accessible to all, regardless of age, income, or social circumstances. Following an extensive selection process, Innovaccer was chosen to implement its leading Healthcare AI Platform, providing clinicians with a comprehensive 360-degree view of patients within San Mateo County Health.

The partnership with Innovaccer will focus on managing data for approximately 165,000 active lives and 500,000 legacy lives. By modernizing their technology strategy, SMCH will incorporate Innovaccer’s InNote solution into their Electronic Health Record (EHR) systems. This integration will enable healthcare providers to easily search for patients across multiple EHR systems, review comprehensive medical records, and transition data back into their main EHR platform with a single click.

Innovaccer will provide SMCH with a comprehensive healthcare data platform that aggregates patient data from multiple sources, including EHRs, Health Information Technology (HIT) systems, and third-party community data. This integration enables the creation of unified patient records, empowering providers with a longitudinal view of patient health. The platform’s rich analytics and customizable dashboards will provide actionable insights into patient data, quality, risk, cost, utilization, and provider performance. These insights will support SMCH in achieving their clinical and financial objectives for population health management programs.

Through this partnership, SMCH aims to deliver high-quality care by leveraging a complete dataset for more informed decision-making. The integrated approach facilitated by Innovaccer’s Healthcare AI Platform and InNote solution will significantly enhance population health management and clinical outcomes, ultimately benefiting the entire San Mateo County community.
