PointClickCare & C3HIE: Leveraging ADT & SNF Data for Improved Care Coordination

–              PointClickCare, a leading healthcare technology platform enabling meaningful care collaboration and real-time patient insights, and C3HIE, a community partner offering additional critical interoperability and care coordination services, announced today its first direct partnership.

–              Together, the organizations are working towards a goal of improving health outcomes for patients across the state of Texas and ensuring a more connected care experience at every stage of the patient healthcare journey. 

Enhancing Care Coordination Through Strategic Data Integration: A Collaboration Between C3HIE and PointClickCare

This collaboration will facilitate smoother and safer care transitions by integrating C3HIE’s admission, discharge, and transfer (ADT) data into PointClickCare’s comprehensive platform and incorporating PointClickCare’s extensive Skilled Nursing Facility (SNF) data into the C3HIE network. PointClickCare has a longstanding history of partnering with C3HIE, including a joint effort with the Texas Health Services Authority (THSA) in 2022, which expanded the Emergency Department Encounter Notification (EDEN) network. This expansion incorporated over 100 Texas facilities, significantly enhancing care coordination by contributing encounter notification data.

In recent months, PointClickCare has significantly expanded its robust care collaboration network, resulting in a threefold increase in hospital data across Texas. This growth has amplified the need for real-time notifications to providers and care managers to improve care coordination and delivery. As part of this collaboration, C3HIE will integrate more than 40 hospitals into the PointClickCare platform, providing greater access to real-time data and care insights, and further enhancing interoperability among care teams.
