Seamless Care Transitions: A Focus on the Patient Journey
This integration aims to streamline patient care by implementing three key Bamboo Health solutions: Pings, Spotlights, and Discharge Summaries. These tools will be used across Prisma Health’s network of 20 hospitals, 835 practice locations, and over 5,500 clinicians, ensuring a consistent standard of care for patients transitioning between different healthcare settings.
