As part of the implementation, Tandigm will utilize athenahealth Population Health for care management as well as analytics, risk stratification, and quality management to help facilitate patient-centered, value-based care. athenahealth’s network-enabled service will coordinate care on a common platform for the patients in the Tandigm network and increase visibility to help the care team address patient needs and rigorously pursue quality outcomes.
