Specialty Pharmacy Launches Care Management App Powered by Patient IO

Diplomat, the nation’s largest independent specialty pharmacy launches AllMetRx, a new care management app powered by Patient IO. With AllMetRx™, real-time treatment data is captured by caregivers and patients to monitor adherence, side effects, and other pertinent clinical data which is shared with providers and stakeholders.

The care coordination solution that can be integrated with any healthcare software system, including electronic health records and population health management programs. Patient IO helps healthcare organizations coordinate care for patients between visits, enabling more efficient, personalized care management and improved patient outcomes.

For patients and caregivers, AllMetRx™ turns a care plan into simple daily tasks, making it easy to follow treatment-specific instructions, securely message with their care team, and read educational content.
