Atlanta-based population health solution provider Wellcentive, has been announced as winner of the Health IT Leadership Summit’s annual Phoenix Awards for the category of “Emerging Company of the Year.”  Presented annually by Metro Atlanta Chamber in conjunction with the Health IT Leadership Summit, the Phoenix Awards recognize individuals and companies that have made an outstanding contribution to the growth of the HIT industry in Georgia.

To date, customers receive more than $500 million annually in value-based revenue by improving outcomes for 30 million patients, using intelligence gained from 1 billion customer data points each month.

Wellcentive has customers in all 50 states, helping to improve outcomes for 30 million patient lives. Its clients comprise physicians, hospitals and health systems, employers and payers, including Borgess Health (Mich.), Evergreen Health (Wash.), Children’s Health Alliance (Ore.), MGM Resorts International (Nev.), CHRISTUS Health (Texas) and Delaware Valley Accountable Care Organization (Pa.).

“We are honored to accept the Health IT Leadership Summit’s award—a true testament to our entire team and their dedicated commitment to driving quality improvement, revenue growth and business transformation for healthcare organizations moving to value-based care,” said Tom Zajac, Wellcentive CEO. “We’re equally grateful to be based in the Atlanta area where we can contribute  to the nation’s  No. 1 growth and innovation hub for health information technology work.”
