Caradigm, a provider of enterprise population health, today announced that Continuum Health Alliance, a physician enablement company providing practice management, population health and community care services to provider groups, has selected Caradigm as part of its population health strategy.
Continuum provides a next-generation health platform, enabling independent physician organizations, hospital-based physician groups, commercial accountable care organizations (ACOs) and MSSP ACOs to achieve the Triple Aim; improve quality of care; enhance the patient experience; and lower the overall cost of care. Continuum will add Caradigm to its integrated health platform, combining Continuum’s existing best-of-breed technologies, care management best practices and expertise with Caradigm’s population health solutions to scale its managed care services business as it assumes an increasing number of value-based contracts.
Continuum will implement Caradigm® Care Management, Quality Improvement and Risk Management solutions as well as the Caradigm Intelligence Platform (CIP) and Knowledge Hub. These solutions support the intelligent integration of data and provide Continuum with additional tools required to provide visibility into the progression of patient populations, so that gaps of care can be identified and acted upon.
