TriZetto and Wellcentive partnership offers advanced solutions for collaborative care.

TriZetto and Wellcentive, Inc. today announced a partnership to offer solutions that enable payers and providers to more effectively collaborate through accountable care initiatives to drive improved clinical and financial outcomes.
TriZetto’s newly launched collaborative care solution, powered by Wellcentive, will provide actionable intelligence to execute highly-impactful accountable care programs for improving both clinical and financial outcomes.

By aggregating and normalizing data from multiple clinical and administrative systems, the new solution facilitates coordinated, provider-led chronic condition management and preventive care, while offering views of health status at the population, practice, and patient level.
