CRM leader Salesforce has added two major features to Salesforce Health Cloud—Health Cloud Empower and Concurrent Care Plans—that enable healthcare professionals to better care for patients, whether at their homes or in clinical settings, and across multiple care plans. Salesforce Health Cloud is a cloud-based patient relationship management solution that allows health providers, payers and life science companies to put patients at the center of their care.
Patients today expect to interact with their health providers in the same way they use other consumer services, such as making mobile payments or chatting with service representatives online. In fact, 59% of Americans would choose a primary care doctor who offers a mobile app over one who does not, and 62% say they would be open to virtual care or in-home treatments. At the same time, many patients have comorbid conditions, which require unique care plans for each medical issue—meaning providers are under pressure not only to communicate to patients in modern ways, but also to create highly personalized care plans across multiple care teams.
The two new features in Health Cloud that allow health professionals to better manage care anywhere and to collaborate across a range of different care plans for individual patients:
· Health Cloud Empower extends Health Cloud beyond the doctor’s office, enabling care providers—whether care coordinators, home nurses, hospice care or family members—to engage with broader care teams from any device. Health Cloud Empower includes Care Team Lists, which allow caregivers to easily view and communicate with the entire care team; Care Plan Selector, which helps caregivers assign care plans to patients; and Care Plan Tasks, which enables care providers to assign tasks to patients while on-the-go.
· Concurrent Care Plans in Health Cloud allow multiple teams, even with disparate workflows and expertise, to collaborate more efficiently, better understand patients and build stronger relationships across care teams. For comorbid patients, care providers can now assign patient multiple care plans to different care teams, which provides more personalized care journeys. And by assigning permissions, providers can control who has access to patient data, keeping personal medical information secure.
Pricing for Salesforce Health Cloud starts at per user, per month. Concurrent Care Plans and Health Cloud Empower are included with a Health Cloud license. To use Empower components in a community, organizations must purchase a Community Cloud license (pricing based on volume of usage).
In addition to these new features, Health Cloud partners are also extending the power of Health Cloud in new ways. myStrength’s Digital Behavioral Health Resources app is now generally available, providing evidence-based tools and activities for behavioral healthcare. And RELATIENT’s RideToHealth app is also generally available, leveraging a unique partnership with Uber to book and manage Uber rides to and from healthcare facilities. Both of these apps can be installed directly within Health Cloud and are available on the Salesforce AppExchange, the world’s largest and longest-running enterprise apps marketplace.
