Centra, an integrated delivery network based in Virginia has selected Cerner to implement its Millennium EHR across its five hospitals, more than 50 ambulatory and long-term care facilities, and health plan. Additionally, Centra will also implement Cerner’s near real-time population health management platform, HealtheIntentSM.
The platform aggregates data from multiple sources into a single patient record allowing Centra to match the right care programs to the right individuals across its patient and community populations. Centra will leverage HealtheIntent for its health plan that currently covers more than 45,000 individuals to facilitate care coordination across health plan members.
