Health First, a Central Florida not-for-profit, integrated delivery network is teaming up with HealthLoop, a patient engagement platform to establish automated communications with their patients, starting with surgeons in the Orthopedic Surgery Department at Health First’s Holmes Regional Medical Center.
With HealthLoop, Health First patient follow ups will be automated and through text message and email communications. Physicians will be able to deliver timely, condition-specific guidance and reminders, while also addressing clinical questions and providing educational materials to help keep patients on track, detect impending clinical concerns and provide a 5-star patient experience.
“Health First is once again leading the charge in integrated health delivery,” said Todd Johnson, CEO of HealthLoop. “As other hospitals are scrambling to prepare for the CJR value-based payment program this spring, the team at Health First is already applying technology to help their orthopedic surgeons coordinate care and catch adverse reactions post-surgery. There’s no doubt – with HealthLoop on board – that they’ll be ready for April.”
Developed by physician specialists in 2009 , HealthLoop automates patient engagement to help health providers reduce cost, improve outcomes and deliver a 5-star patient experience. Reassuring guidance each step of the way connects care teams with patients who need it most. HealthLoop enables value-based care for hospitals to monitor at-risk populations, provide outreach when most needed and track patients through an episode of care. Patients rate their doctors with a 5-star experience and providers realize better efficiencies with an automated patient check-in approach. The company has raised over $15M in funding to date led by Canvas Ventures.
